A thyroglossal duct cyst (TGDC) is a common congenital anomaly of the neck that occurs when the thyroglossal duct, which forms during fetal development to guide the thyroid gland from the base of the tongue to its final location in the neck, fails to completely obliterate after the thyroid gland descends. This remnant duct can form a cyst, which is typically located in the midline of the neck, often just below the hyoid bone. TGDCs are usually benign, but they can sometimes become infected or cause other complications.

Symptoms

Most TGDCs are asymptomatic, meaning they do not cause any noticeable symptoms. However, some people with TGDCs may experience:

  • Difficulty swallowing
  • Pain or discomfort in the neck

  • Coughing
  • Shortness of breath

Diagnosis

TGDCs are typically diagnosed based on a physical examination and medical history. Your doctor may also Free Download one or more of the following tests to confirm the diagnosis:

  • Ultrasound: This imaging test uses sound waves to create images of the inside of your neck. It can help to identify the size, location, and contents of a TGDC.
  • CT scan: This imaging test uses X-rays and computers to create detailed images of the inside of your neck. It can help to identify the size, location, and extent of a TGDC.
  • MRI scan: This imaging test uses magnets and radio waves to create detailed images of the inside of your neck. It can help to identify the size, location, and extent of a TGDC, as well as any surrounding structures that may be affected.

Treatment

The treatment for a TGDC depends on the size, location, and severity of the cyst. Small, asymptomatic cysts may not require treatment. However, larger cysts or cysts that are causing symptoms may need to be removed surgically.

Surgery to remove a TGDC is typically performed through an incision in the midline of the neck. The surgeon will carefully dissect the cyst from the surrounding tissues and remove it. In some cases, the surgeon may also need to remove a portion of the thyroglossal duct.

Complications

Most TGDCs are benign and do not cause any complications. However, in some cases, a TGDC may become infected or cause other complications, such as:

  • Abscess: An abscess is a collection of pus that can form in a TGDC. It can cause pain, swelling, redness, and fever.
  • Fistula: A fistula is an abnormal connection between two organs or body cavities. It can develop between a TGDC and the skin, or between a TGDC and the trachea.
  • Thyroid dysfunction: A TGDC can sometimes compress the thyroid gland and interfere with its function. This can lead to hypothyroidism, which is a condition in which the thyroid gland does not produce enough thyroid hormone.

Outlook

The outlook for people with TGDCs is generally good. With proper treatment, most people can live normal, healthy lives. However, it is important to follow your doctor's instructions carefully to prevent complications.
